Travel Risk Assessment

Please note at Wootton Vale Healthy Living Centre & Shortstown Surgery we only offer vaccinations for travel that are available on the NHS.

We do not offer full travel consultations with advice on vaccinations, malaria treatments and other recommendations that you may need for the country you are travelling to. It is your responsibility to book into a private travel clinic to access this information.

travelhealthpro.org.uk will help you to assess the vaccinations and malaria treatments you may require.

If you need to research further, please visit UK Malaria Guidelines for travellers from the UK.

You should consider all vaccinations should be arranged 6 weeks prior to travelling. It is your responsibility to book early prior to travel.

You can be entitled to the following vaccinations on the NHS service – all other vaccinations are private, and we do not administer them here – it is your responsibility to arrange these.

  • Tetanus
  • Diphtheria
  • Polio
  • Pertussis
  • Hepatitis A
  • Typhoid
  • Measles/Mumps/Rubella if required at not already received two vaccinations in your life

At travelhealthpro.org.uk you will also find information about:

  • Risks of Zika – please note the countries that are at risk of Zika. If you are considering getting pregnant or are pregnant the advice is not to travel, there.
  • Risk of Dengue – transmitted via mosquitos, although rare can be life threatening – assess you risks prior to travel.

Due to COVID-19, travel advice is subject to rapid change. Countries may change entry requirements and close their borders at very short notice. Travellers must ensure they check current Foreign, Commonwealth & Development Office (FCDO) travel advice https://www.gov.uk/guidance/travel-abroad-from-england-during-coronavirus-covid-19

In addition, check the FCDO specific country page: www.gov.uk/foreign-travel-advice

(where available) which provides additional information on travel restrictions and entry requirements, safety, and security advice.

All travellers should ensure they have adequate travel insurance.

You will need to complete the following form for every individual member of your family travelling if you require us to administer NHS vaccinations or are unsure if you require them.
